Professional drivers 60+, including taxi drivers, bus drivers, and heavy equipment operators are required to undergo health checkups and eye tests. All other drivers are required only from age 70.
The results of the eye checkup must be signed by an ophthalmologist or optometrist. See the form here.
If relevant, one should receive a health form in the mail about 6 months before the current license expires. If it wasn’t received but is required, the driver should call *5678, or download the form online (scroll down to Health checkups for drivers over 60), or pick one up from your local Misrad HaRishui.
Have your doctor complete and sign the form and submit it to Misrad HaTachboera along with your other documents. There are two options:
in English:
Misrad HaTachbura
The Licensing Division, Update and Control Department
P.O Box 270
Holon 5810201
or in Hebrew:

Note: If your license is about to expire (within about one month), and you haven’t received a reply regarding your health checkups, you can request a temporary license at a licensing office. It’ll be valid for 6 months.
